FORMS FOR DOCTOR'S VISITS
If a student athlete sees a physician regarding an injury, they must get this medical release form signed by their doctor in order to continue participating in their sport.

FORMS FOR PHYSICALS
All forms MUST be completed BEFORE a student can participate
in athletics before, during, or after school.

DISTRICWIDE PHYSICALS
HIGH SCHOOL PHYSICALS will be Saturday, Aug. 1 at Vista Ridge High School. Enter at the back of the athletic area by the football fields. Cost is $20. (Make checks out to LISD). There are new forms that must be completed. Old forms will NOT be accepted.

Echocardiograms will be provided by David Hope of Texas Heart and Vascular. An echocardiogram screening can detect hypertrophic cardiomyopathy (enlarged heart) and certain underlying heart conditions that can lead to sudden cardiac death. This screening is not required for the pre-participation physical examination. Checks MUST be separate from Physical cost and made out to: David Hope for $10.  Waiver forms MUST be completed.


THESE PHYSICALS ARE ONLY OPEN TO LISD HIGH SCHOOL ATHLETES
AND OTHER UIL PARTICIPANTS (BAND, DANCE, CHEERLEADING, ETC.)

BOYS
8 a.m. Boys 9th and 10th grade
9:30 a.m. Boys 11th and 12th grade

GIRLS (we recommend wearing a swimsuit)
11 a.m. Girls 9th and 10th grade
12 p.m. Girls 11th and 12th grade

All athletic paperwork and physicals must be turned to the Athletic Training Room BEFORE athletes can participate in a sport. You must use the UIL and athletic forms for LISD. Forms are in the athletic training room or you can download them.
